Deviated Nose – in consideration of External Rhinoplasty

We perform many surgeries for deviated nose, most of which are accompanied by external nasal deformity.
It is a new surgical procedure in Japan called "External Rhinoplasty of the Nasal Septum".
We have performed hundreds of cases since my university days.
External Nasal Septum Rhinoplasty is performed by dividing the external nose into a roof, a pillar and a wall.
The curved wall (nasal septum) is removed while preserving the roof and pillars, but in cases where the roof and pillars are curved, the removed wall is made into plates and then crossed, so the roof and pillars are straightened.
This is the standard technique.
However, it has been said that even if the plates are used to straighten, the tendency of the bent has not been totally removed, therefore leaving a risk of another deviation.
We have applied the technique of "Preservation Rhinoplasty", which is actively practiced in Europe and the United States, to the Japanese. We not only correct the bent pillar and the roof with by using the plates, but also cut them away from the foundation, then cut them back to the position where the tendency of the bent is removed and finally put them back on the foundation as needed.
In Japan, rhinoplasty is often performed jointly with otolaryngology, but when looking around the world, it is standard for a single surgeon to perform the procedure.
We believe that this is due to the fact that the nasal septum is manipulated as described above in consideration of the external nose, so it is easier to perform by a single surgeon who can assume the correct outcome of the surgery.
